Where can you apply?
You can apply for a birth certificate at the local ward office of MCGM with the Medical Officer for Health (MOH). Download the application form here.
How can I check my records?
Birth records after the year 1996 are available in the computer system at the ward office, but the records prior to 1996 may be available in the physical files at their centralized office in Mazgaon.
Please input the Registration Number, a number provided to you by the Hospital/ Nursing Home/ Maternity home; for searching your Registration record in the system at mcgm.gov.in. In case you are unable to put the registration no. you may provide the other search criteria such as Gender, Date of Birth, Ward, Father’s Name, Mother’s Name or Hospital Name.
What is the procedure for naming my child?
In case you want to name your child, first, enter the additional data to authenticate your identity. You may then input any one of the fields i.e. PAN Card no., Driving License No., Voter’s card No., Passport No., Ration Card No. or Mother’s maiden Name. Please ensure that the data provided by you matches the information provided at the time of registration. Note: A child’s name once entered and saved cannot be changed. To avoid this please confirm the name and other details you would like to enter in the form before submitting the form.
Where do I have to collect the birth certificate from?
If you have opted to take the delivery of certificate through courier, the same will reach you within the days stipulated by the Department of Posts. Please re-confirm the Delivery address before submitting the form in case of mode of delivery – courier. Or you can collect the same from the ward office.
Can i apply online?
You can apply online at the MCGM website and make online payment by using Net banking facility from the listed banks through the Payment gateway. Once the payment is made online, please make a note of transaction id that is generated.
